Service Summary
Intensive Case Management Service Summary In addition to the community programs, the hospital also provides the Regional Intensive Case Management Program serving mental health and addiction clients throughout Huron and Perth Counties. Intensive Case Management services provide time limited goal specific in-home community treatment. The program focusses on relapse prevention and stabilization in the community utilizing psychological education, medication management, liaising with partner agencies and advocacy.
